To apply for child care or to learn more about Family Day child care programs and availability in your area, you may

1. complete this online application form, or
2. contact our Child Care Services Department at 416-922-3434 / 1-800-263-9703 during business hours (Monday-Friday, 8:30 a.m. - 4:30 p.m.) and provide the information over the phone.

The information collected on this form is used only for processing your application for child care with Family Day Care Services.
